What is a PCA:

-A Personal Care Assistant (PCA) is a trained and certified individual who helps with day to day activities in the home and community.

What services do ReliaCare Home Health provide:

-PCA’s help with activities of daily living (ADLs), health related procedures and tasks, observation and redirection of behaviors and instrumental activities of daily living (AIDLs) for adults.
-Example: Housekeeping, meal preparation, eating, bathing, dressing, hygiene, toileting, grooming, companionship, medication reminders, transfers, accompanying to and from doctor visits.

How do I qualify and who pays for PCA Services:

-In order to qualify for PCA Services, you must be eligible and enrolled in a Minnesota Health Care Program that includes PCA Services. PCA Services are available to persons covered by Medical Assistance (MA) and Minnesota Care expanded benefit set. The majority of PCA services are paid by Medical Assistance. -Next you must have an assessment by a lead agency public health nurse to determine what services you are eligible for. Contact us at 612-366-1381 if you need help requesting a PCA assessment.

Can I use a relative for services:

-Yes, but all PCA’s must complete training through Minnesota Department of Human Services (DHS) Individualized PCA Training. This training is free of charge and is available on line. Once the PCA has passed the required training, please fax a copy of the certificate to 651-298-0972.

How do I transfer from my current PCA company to ReliaCare Home Health:
